Custom forms enable you to tailor SOAP notes and patient health forms to your practice's specific needs. Whether you specialize in Chinese medicine, chiropractic, or another field, this guide walks you through creating, editing, and automating these powerful tools for a streamlined workflow.
Managing Custom forms
Accessing Custom forms:
Navigate to Settings > Notes > Custom forms to view and manage your forms.
Existing options include default SOAP format forms (Subjective, Objective, Assessment, Plan) or your created forms.
Editing forms:
Select an forms to edit or preview, or create a duplicate to preserve the original.
Customize fields such as labels, placeholders, order, and type (e.g., checkboxes, text inputs).
Creating New forms:
Click Add Custom forms for a blank template.
Use the side tray tools to build sections with text, multiple-choice options, severity scales, or numeric units.
Drag fields into place, set visibility for PDFs, and save your changes before leaving the page.
Automation and Integration:
Pair forms with specific services (e.g., a Women's Fertility Form linked to an herbal consultation).
Automate reminders for incomplete forms or recurring visits.
Integrate forms directly into SOAP notes or patient health forms for streamlined documentation.
Form Distribution:
Use the Patient Portal to send forms or custom forms directly to patients.
Track completion status, send reminders, and access form history from the portal or schedule interface.
